Job summary

We seek an enthusiastic and driven individual keen to develop and extend the role of pharmacists within primary care. The role will be patient facing and include all aspects of medicines management across both long-term conditions and acute presentations. You will work as part of the highly-skilled clinical team guiding them in innovative and cost-effective prescribing.

Main duties of the job

  • Structured Medications Reviews
  • Assisting the Prescription team with queries
  • Attending regular in-house MDTs
  • Working with the ICB Medications Management Team to ensure the local formulary is adhered to and understood in house
  • Carrying out audits when required
  • Helping deliver the targets laid out in the local Prescribing Quality Scheme
  • Helping deliver the targets laid out in QOF and IIF
  • Working with our care homes to ensure optimal prescribing
  • Advising GPs and other clinical staff
  • Helping develop and maintain policies and protocols relating to medicine management
