Summary
The Learner Journey Coordinator plays a key role in supporting young people with SEND to achieve positive outcomes and successful transitions into specialist education, guiding learners from initial enquiry through to enrolment. The role requires an organised, proactive and relationship-focused approach to ensure a positive learner journey.
Working collaboratively with the Admissions and School Partnership Coordinator, Curriculum Managers, and the Careers, Employment & Futures Manager, the Learner Journey Coordinator will help ensure that learners' needs, aspirations, support requirements and intended outcomes are clearly identified throughout the admissions process. Coordinate EHCP reviews, gather and analyse information relating to learner needs, support funding applications and audits, and maintain accurate records in line with the SEND Code of Practice.

Key Responsibilities
Admissions and Consultations
- Support the Admissions and School Partnership Coordinator to ensure the smooth process from enquiry through to enrolment.
- Review EHCPs, professional reports and supporting documentation to assess learner suitability for college programmes.
- Support the Admissions and School Partnership Coordinator to facilitate admissions assessments, transition visits and applicant interviews.
- Support the Admissions and School Partnership Coordinator to maintain accurate admissions records and application tracking systems.
- Liaise with Curriculum Managers and support teams to determine placement suitability and capacity.
- Support college open days, transition events and learner visits.
EHCP Coordination
- Coordinate annual reviews and interim reviews in line with statutory requirements.
- Attend the EHCP reviews and gather information from learners, families, curriculum staff, employers and external professionals.
- Produce high-quality review documentation and submit information within required timescales.
- Maintain oversight of EHCP outcomes, provision requirements and support arrangements.
- Ensure learner records remain compliant with SEND legislation and Local Authority requirements.
- Support the Assistant Principal of Behaviour, Inclusion and Safeguarding to complete funding applications and evidence requests.
Transitions and Learner Journey
- Support the College team to ensure effective transitions into, within and out of college.
- Support the Careers, Employment & Futures Manager to develop transition plans in partnership with learners, families and external agencies.
- Support the Careers, Employment & Futures Manager to liaise with schools, colleges, adult services and employers to support progression planning.
- Ensure learner information is shared appropriately to facilitate successful starts and transitions.
- Support the Careers, Employment & Futures Manager to maintain destination tracking and progression reporting.
Stakeholder Engagement.
- Develop positive working relationships with learners, parents/carers, Local Authorities and external professionals.
- Act as a contact for EHCP-related enquiries.
- Attend multi-agency meetings as and when required.
- Provide guidance to staff regarding learner needs, EHCP outcomes and support requirements.
Quality and Compliance
- Ensure all admissions, EHCP and learner information is maintained accurately and securely.
- Maintain compliance with the SEND Code of Practice, GDPR and safeguarding requirements.
- Contribute to quality assurance activities and internal audits.
- Monitor and evaluate admissions and transition processes and contribute to service improvements.
Alongside supporting the coordination of the admissions, transitions and EHCP processes, supporting the college team to maintain oversight of learner engagement, attendance and progress, coordinating early intervention strategies and contributing to STAR meetings and learner reviews. The role will ensure that learner needs are identified proactively, support arrangements remain effective and barriers to learning are addressed in a timely manner, enabling learners to make sustained progress towards their EHCP outcomes, educational goals and long-term aspirations.

Desirable Skills and Experience
- Experience of coordinating admissions, learner assessments or transition processes.
- Knowledge of the SEND Code of Practice and EHCP statutory processes.
- Experience of working with Local Authorities and external agencies.
- Ability to analyse EHCPs and professional reports to identify provision requirements.
- Experience of maintaining accurate learner records and databases.
- Excellent report-writing and administrative skills.
