- You have current registration with Social Work England
- You have completed your AYSE
- You have an in-depth knowledge of current legislation and guidance, as well as current national policy and practice developments
- You have a full Drivers Licence and the ability to travel between different locations

At Peterborough City Council's Transfer of Care Team, we believe in the power of change and the importance of community. We’ve got some fantastic opportunities due to recent promotions, and we’re on the lookout for enthusiastic social workers to join our friendly, fun, and cake-loving team! Our team provides essential support for adult service users, ensuring safe and timely discharges from hospital. Working closely with Peterborough City Hospitals multi-disciplinary teams, you will enable discharges within the Discharge to Assess framework.
