If so, then this is your chance to join our Health and Social Care Partnership as a Social Care Officer, in the Supported Living Team, within Perth and Kinross.

\\n

We are looking for a motivated and enthusiastic person to join our team supporting adults with a learning disability, mental health needs and autism within their homes.

\\n

The role will predominantly focus on working within a smaller team with a young adult with learning difficulties and autism providing personalised care and support to enhance his wellbeing and quality of life.

\\n

You will work collaboratively with individuals, their families and healthcare professionals.

\\n

You will support individuals to develop skills to promote their independence and achieve their goals.

\\n

You will be required to maintain accurate records and reports, adhere to guidelines and protocols ensuring best practice and safeguarding.

\\n

We work on a 6 week rolling rota pattern between the hours of 8am and 9pm.

\\n

As a successful post holder, you will be responsible for:

\\n

- Following a planned schedule of service user visits.

\\n

- Supporting service users to maintain their own home.

\\n

- Supporting service users based on their assessed needs.

\\n

- Supporting service users with personal care tasks when required to enable the person to reach their optimum level of independence.

\\n

- Monitoring health & wellbeing and supporting to appointments.

\\n

- Supporting service users to access community facilities / activities.

\\n

- Supporting service users to recognise and achieve their potential.

\\n

- Protecting individuals from risk or abuse and to report any concerns through appropriate process.

\\n

- Ensuing Health & Safety for yourself and service users' in line with PKC policy and procedures.

\\n

- Participating in assessments.

\\n

- Maintaining accurate records and reporting on service users' progress.

\\n

- Complying with the National Care Standards and SSSC Codes of Practice.

Requirements

You should be actively fit, committed to person-centred practice, the ability to work flexibly to respond to the needs of the service and good communication and inter-personal skills.

Experience of working with individuals with autism would be an advantage. 

\\n

You will be required to register with the Scottish Social Services Council (SSSC). As part of this registration commitment, you will undertake your own personal development and work towards recognised qualification.

\\n

There is no entry qualification requirement, but you will be required and supported to work towards SVQ in Social Services and Healthcare at SCQF level 7 (was SVQ3).

\\n

A valid UK driving licence is desirable but not essential.

\\n
