We are currently recruiting an experienced Physiotherapist to join a Reablement Service in Haringey. This is an excellent opportunity to work within a fast-paced, multidisciplinary environment supporting timely hospital discharges and promoting independence within the community.
Key Responsibilities:
- Provide holistic initial assessments for service users with complex care and therapy needs within the Reablement team.
- Support service users to set client-centred goals that promote independence and wellbeing in the home environment.
- Assess, progress, and monitor mobility and balance, using standard physiotherapy outcome measures to evaluate treatment effectiveness.
- Assess and provide equipment to support independence with activities of daily living (ADLs).
- Undertake comprehensive falls assessments and implement appropriate intervention plans.
- Assess transfers to reduce the risk of injury to service users and carers and, where feasible, support the reduction of care packages (including moving from double-handed to single-handed care).
- Support the Duty Manager by triaging referrals when required, ensuring appropriate pathways into Adult Social Care.
- Carry out urgent home assessments to facilitate timely hospital discharges or respond to functional deterioration in the community.
Requirements:
- Qualified Physiotherapist with relevant professional registration.
- Experience working within community services, social care, or the NHS.
- Strong assessment, clinical reasoning, and care-planning skills.
- Ability to work independently while contributing effectively within a multidisciplinary team.
