Responsibilities
- Manage urgent referrals within the Connect Team triage service
- Gather relevant information to support decision-making
- Complete functional assessments and develop support plans
- Undertake moving and handling assessments
- Carry out home visits for assessments and follow-up work
- Work collaboratively with multi-disciplinary colleagues
Working Arrangements
- Full-time post (35 hours per week)
- Community-based role with an element of hybrid working
- Monday to Friday working pattern
