We are seeking 2 experienced agency Social Workers to join the Transfer of Care Hub at Warrington hospital. This is a fast-paced, hybrid role supporting safe and timely discharges from hospital. You will work collaboratively with health and multidisciplinary partners to assess needs and reduce delayed transfers.
The post is full time, 37 hours per week, with a requirement to base in the office around 3 days per week. Recent experience in hospital or Transfer of Care setting is essential, along with strong assessment, communication and decision making skills. Knowledge of the social care system Mosaic is desirable.
