What you need:
- A degree in Social Work or equivalent
- Registered with Social Care Wales/ Social Work England
- Previous experience within a Transitions Teamor a similar Young adult social care settingis essential
- Completed relevant post-qualifying training
- Enhanced DBS check
- Excellent communication and written skills
