The Reablement team positively impacts individuals' lives following illness, injury, loss of confidence or hospital admission. The service is led by Occupational Therapists, alongside Occupational Therapy Assistants and Support Workers. Our OTs work proactively with customers in their own homes to achieve greater independence, promote their well-being and minimise long-term care needs. In this placement, you will conduct strengths-based assessments, and work with customers and their families to design and implement creative reablement plans to enable them to achieve their goals. We’re looking for a qualified OT with at least two years of post-qualification experience. Holding a valid full UK driver's license is required, as they'll be conducting visits to clients in their own homes or other settings, such as hospitals.
