Job Overview: We’re looking for an enthusiastic and proactive Band 6 Rotational clinical Pharmacist to join this hospital’s pharmacy team. This is a fantastic opportunity to gain valuable experience across a range of clinical specialties, develop your clinical skills, and make a real impact on patient care. In this role, you will provide ward-based pharmacy support, working closely with the multidisciplinary team to ensure the safe and effective use of medicines throughout the patient journey.
Key Responsibilities
- Providing clinical pharmacy services to inpatient wards, including medicines reconciliation and therapeutic reviews.
- Clinically screening prescriptions to ensure appropriateness, safety, and adherence to Trust guidelines.
- Ordering medications to ensure timely supply and avoid treatment delays.
- Identifying and resolving medicines-related issues in collaboration with doctors, nurses, and other healthcare professionals.
- Supporting discharge planning through timely review and supply of take-home medications.
- Contributing to the delivery of high-quality patient care and continuous service improvement.
