We are looking for a proactive and experienced Band 8a Pharmacist to lead and support pharmacy services across the inpatient and outpatient dispensaries and general ward areas for this NHS Trust. This is an exciting opportunity to use your clinical expertise and leadership skills to enhance patient care, support the pharmacy team, and contribute to service development.
Key Responsibilities
- Clinical screening of prescriptions to ensure safe, effective, and evidence-based use of medicines
- Conducting final accuracy checks (ACT) for dispensed items in accordance with legal and professional standards
- Providing high-quality patient counselling to support medicine optimisation and promote adherence
- Delivering ward-based pharmacy services, including medicines reconciliation, interventions, and discharge planning
- Working collaboratively with multidisciplinary teams to ensure seamless pharmaceutical care
Essential Requirements
- GPhC registered pharmacist
- Previous experience in hospital pharmacy
- Strong clinical knowledge and confidence in decision making
- Excellent communication, organisational and leadership skills
- Commitment to patient-centred care and continuous service improvement
