Overview

Role Summary: The Quality Improvement Investigator with the support of the Assistant Director of QI is responsible for reviewing, investigating, and documenting incidents within the agency. They will assist with the implementation of agency-wide policies, procedures and training to ensure compliance with all state and federal regulations and guidelines pertaining to incident management

Responsibilities

  • Conducts, coordinates, and documents incident investigations.
  • Reviews incident documents, communicating with and making recommendations to Directors, Supervisors, other Investigators and agency Administration.
  • Compiles, organizes, and distributes incident packets and reports.
  • Creates and maintains spreadsheets pertaining to incident management.
  • Represents the department at meetings and trainings pertaining to Incidents or Quality Management issues and/or initiatives.
  • Assists with ongoing incident training throughout the agency.
  • Assists with the development of the annual Trend Report, to include both reportable and non-reportable incidents.

Qualifications

  • SALARY 26.10-27.47
  • Bachelor’s Degree required in human services or related field
  • 3+ years’ experience working in OPWDD setting OR 5+ years of experience working directly as an Incident Investigator
  • Must have Incident Investigator certification or be able to complete course and pass certification exam
  • Extensive knowledge of Microsoft Office software
  • Experience working with OPWDD’s incident management software (IRMA) preferred
  • Must be able to display a high level of discretion and understand the sensitive nature of confidential information with superior emotional intelligence.
  • Valid NYS Driver’s License
