The Role: Are you passionate about helping people stay independent and live life to the fullest? In this role, you’ll support individuals aged 65 and over to remain in their own homes with confidence and dignity. You’ll provide practical help with benefit checks, forms, Blue Badge applications, and grants. You’ll also visit people in their homes, assess risks, and connect them with services such as Falls Prevention, Occupational Therapy, and the Fire Service. Alongside this, you’ll share up-to-date advice on care options, social activities, and housing so individuals can make informed choices about their future. Accurate electronic record-keeping and effective case management will be part of your daily routine. You’ll work as part of a supportive team, attending meetings, training, and regular supervisions.
