Our Individual Support Program is looking for compassionate people to serve as an IS Counselor providing premier services to the individuals in our care.
What does the job entail?
- You’ll be supporting adults with developmental disabilities in individuals' homes.
- Implement individualized programs as specified in Individual Service Plans.
- Provide for individuals’ health and welfare.
- Ensure general cleanliness and safety in individuals’ homes.
- Providing individual community outings.
- Encourage rapport with local resources.
- Instruct and assist with meal preparation.
- Assistance with activities of daily living.
- Instruct and physically assist with housekeeping activities.
- Medical appointments.
- Administer and document medication and/or monitor medical health.
- Assist with financial management.
- Ensure home is safe.

As an Individual Support provider, you will provide opportunities for individuals to integrate into the community and enjoy outings that interest them. Examples of some outings are:
- The Movies
- Shopping
- Going to Dinner
- Baseball Game
- Bowling
