Health and Wellbeing Coach (Mental Health)

Here in Penwith Primary Care Network (PCN) we have created a coaching service as part of the transformation of mental health care in Cornwall. There has never been a more exciting time to work in mental health services, especially in primary care. We want to hear from people who have passion and creativity to help shape and build our holistic service in Penzance, Hayle and St Ives.

We aim to create a space where we understand what matters to you, not whats the matter with you. Our new team will include a diverse range of skills, to listen and support people who are struggling with their mental wellbeing. We are in the process of creating a welcoming hub in Penzance & Hayle, with outreach supporting our more rural areas.

Main duties of the job

As a Health and Wellbeing Coach (Mental Health) you will work as part of a team, coaching patients both on a face to face and virtual basis.

This role will be predominantly hub-based, working across multiple locations with some remote working and outreach clinics in Penwith. You will be a valuable member of the mental health & wellbeing team, as well as being aligned to one or more GP practices in the patch. A driving licence is essential.

We are looking for candidates who are enrolled in, willing to undertake, undertaking or qualified from Health Coaching training with the training delivered by a training organisation listed by the Personalised Care Institute. A minimum of 2 years\' experience in a mental health role is desirable.

Job responsibilities

Key responsibilities

Manage and prioritise a caseload, taking an non-judgemental approach based on strong communication and negotiation skills, while considering the whole person when addressing existing issues.

Use IT and MDT channels to screen patients, with an aim to identify those who would most benefit from health coaching.

Provide personalised support to individuals, their families and carers, to support them to be active participants in their own healthcare; empowering them to manage their own health and wellbeing and live independently through:

  • Coaching and motivating patients through multiple sessions to identify their needs, set goals, and supporting patients to achieve their personalised health and care plan objectives.
  • Providing interventions such as self-management education and peer support.
  • Supporting patients to establish and attain goals that are important to them.
  • Supporting personal choice and positive risk-taking while ensuring that patients understand the accountability of their own actions and decisions, thus encouraging the proactive prevention of further illness.
  • Working in partnership with the social prescribing service to connect patients to community-based activities which support them to take increased control of their health and wellbeing.
  • Increasing patient motivation to self-manage and adopt health behaviours.

Work in partnership with patients to support them to develop their level of knowledge, skills and confidence enabling them to engage with their health and wellbeing and subsequently supporting them in shared decision-making conversations.

Utilise health coaching skills to support and develop the knowledge, skills, and confidence to manage their health and wellbeing, whilst increasing their ability to access and utilise community support offers.

Explore and support patient access to a personal health budget where appropriate for their care and support.

Refer people to other health professionals within the PCN as appropriate.

Adheres to a code of ethics and conduct in line with the NHS England health coaching Implementation and Quality Summary Guide.

Wider responsibilities

Develop collaborative relationships and work in partnership with health, social care and community and voluntary sector providers and MDTs to holistically support patients wider health and wellbeing, public health and contributing to the reduction of health inequalities.

Provide education and specialist expertise to PCN staff, supporting them to improve their skills and understanding of personalised care, behavioural approaches and ensuring consistency in the follow up of peoples goals with MDT input.

Raise awareness within the PCN of shared decision-making and decision support tools.

Work closely in partnership with the social prescribing link workers, to work alongside people who may need additional support.

Maintain and update care records, including on GP practice systems.

Qualifications

  • Enrolled in, willing to undertake, undertaking or qualified from health coaching training, with the training delivered by a training organisation listed by the Personalised Care Institute.

Personal Attributes

  • Compassionate, trauma-informed approach.
  • Flexible team player who is equally happy working alone or as part of a team.
  • Ability to work autonomously and make decisions within the boundaries and scope of the role.
  • Willingness to undertake mandatory training online or face-to-face, and to undertake further training as the service/need develops.
  • Driving licence with access to a car.
  • Enhanced DBS check.

Experience

  • Experience of working with people who have trouble coping with their emotions and/or have experienced poor mental wellbeing.
  • Experience of working with people with a wide range of issues, for example drug and alcohol dependence, self-harm, domestic violence.
  • Basic understanding of a range of prevalent mental illnesses, how and why they may develop and the symptoms someone might experience.
  • 2 years\' experience working in a mental healthcare setting.
  • Experience of working with neuro-divergent people.
  • Understanding of health and social inequalities in parts of Penwith.
  • Basic understanding of medication, physiological comorbidities, and the effects of these on a person\'s presentation.
  • Good knowledge of the voluntary sector and charities that are available locally to support bio/psycho/social wellbeing.
