Pennine Care seeks a Band 6 Community Mental Health Nurse to work with older people in Stockport. The role focuses on delivering person-centred community care for adults with severe mental health issues and/or organic diagnoses, across home environments and local clinics.
You will assess, plan, implement and evaluate care in partnership with MDT colleagues, ensuring safe, high-quality practice and timely crisis response where needed.
