Older Adult\’s Home Intensive Treatment Nurse

Pennine Care NHS Foundation Trust is seeking an Older Adult\’s Home Intensive Treatment Nurse to join the Rochdale Home Intensive Treatment Service for Older Age Adults. The service provides an alternative to hospital admission for residents of Heywood, Middleton and Rochdale who are predominantly over the age of 65 and are experiencing a mental health crisis. The team is comprised of Registered Mental Health Nurses, a Team Manager and a Team Administrator.

Responsibilities

  • To undertake holistic mental health and risk assessments using Trust approved documentation and procedures.
  • To support clients who are in mental health crisis who will have a variety of mental health conditions, both organic and functional.
  • To provide evidence based therapeutic interventions promoting enablement, choice and recovery.
  • To comply with all legislative requirements in relation to service users care.
  • Actively participate in multi-disciplinary meetings and contribute to clinical decision making.
  • Engage, supervise and monitor junior staff and student nurses.
