As a Home Health Licensed Practical Nurse (LPN), you will administer skilled nursing care to patients on an intermittent basis in the patient's place of residence in close coordination with other disciplines and caregivers involved in the patient's care. This is performed in accordance with physician orders and plan of care and in accordance with agency policies and procedures.

Qualifications:
- Have a current Pennsylvania Practical Nurse License
- Have previous Home Health experience (preferred)
- Have a valid PA driver's license and reliable transportation
- Have 2 years of LPN acute care or long-term care experience
- Have excellent time management skills
- Have strong verbal and written communication skills
- Have ability to exercise initiative and independent judgement
- Have ability to work independently and within a multidisciplinary team
- Have ability to pass PA Child Abuse History Clearance
- Have ability to pass Criminal Background Check
- Have ability to meet physical requirements of position
