As a Home Health Scheduler, you will coordinate/assign visiting disciplines to patients, including but not limited to: nursing, physical therapy, occupational therapy, speech language pathology, medical social services, dietitian, and home health aides. You will coordinate schedules between multiple providers and disciplines to assure optimal patient care, efficient utilization of services, and compliance with physician orders.

Qualifications
- Have a high school diploma or equivalent
- Have prior home health experience, referral intake, or related work experience in the health care industry
- Have proven ability to interact effectively with individuals at all levels of the organization
- Have strong interpersonal skills
- Have ability to pass PA Child Abuse History Clearance
- Have ability to pass Criminal Background Check
- Have ability to meet physical requirements of position
