Overview
Team Leader – Community Wellbeing & Independence Team at Pembrokeshire County Council. This role involves leading a team to support individuals to live in their own home, including activities of daily living such as personal care, meal preparation and medication. The post requires writing and reviewing care plans, completing risk assessments, and ensuring care standards comply with Social Care Wales and Care Inspectorate Wales.
Responsibilities
- Lead and manage a team delivering high-quality support to individuals in their own homes.
- Write and review care plans and complete risk assessments.
- Ensure service standards are maintained and compliant with Social Care Wales and Care Inspectorate Wales requirements.
- Manage activities of daily living support, including personal care, meal preparation and medication administration.
- Develop and maintain a two-week rolling rota and manage weekend working, with enhancements and every other weekend off.
- Participate in ongoing training and development; provide supervision and leadership to staff; ensure appropriate uniforms and equipment are provided.
- Register with Social Care Wales for the post.
- Maintain high standards of communication and organizational skills; promote the well-being of individuals living in their own homes.
Qualifications
- Minimum of a level 2 in Adults Health & Social Care or equivalent; commitment to work towards level 3.
- Must register with Social Care Wales for this post.
- Good written and verbal communication skills; strong leadership and organizational abilities; positive approach to others’ well-being.
- Provide evidence of right to work in the UK; applicants requiring a Skilled Worker visa will not be considered.
