An exciting opportunity has arisen at Lowri House, Hackney Hospital Discharge Service. We’re looking for someone who genuinely wants to make a difference every day — someone with resilience, adaptability and a strong commitment to customer service and care. Role Summary

We are recruiting a full‑time Support Worker to join our team. We provide specialist housing, care and support to people who are socially excluded, disabled, unwell or homeless. In this role, you’ll deliver high‑quality and effective hospital discharge services that promote independence, wellbeing and inclusion, while working in line with Peabody’s values, policies, procedures and best‑practice standards. You’ll need strong knowledge of support tools and a good understanding of the needs of people being discharged from hospital with complex requirements. Excellent communication and IT skills are essential, along with experience or a solid understanding of the benefits system and move‑on resettlement. Shift Pattern

This service operates 7 days a week on a rota basis.

  • Morning shift: 8:00am - 4:00pm
  • Afternoon shift: 12:00pm - 8:00pm

You’ll be required to work 2 weekends in each month. This role will require an enhanced DBS check.
