Trust Nurse Services is looking for enthusiastic and motivated Community Nurses to support our NHS clients in London. We are offering long-term contracts as well as Ad-Hoc shift work and competitive rates of pay and mileage.

Responsibilities:

  • Coordinating care for individuals with long-term conditions or complex health needs.
  • Administering medication and supporting self-administration.
  • Dressing and managing wounds.
  • Providing Catheter Care in Clinics and within the individual’s home.
  • Providing Wound Care for housebound individuals after surgery.
  • Supporting issues around continence.
  • Cannulation / IV Therapy administration.
  • Caring at end of life.

Experience and Qualifications:

  • 6 months experience working at a relevant level within the community in the UK.
  • Has a full UK driving license and access to an insured car daily.
  • Able to work within a diverse community of patients and healthcare professionals.
  • Effective communication skills written, verbal and non-verbal with the ability to adapt communication for individuals with communication difficulties.
  • Able to make clinically reasoned judgements when working with a variety of conditions.
  • Able to work autonomously, and know when to seek appropriate advice/guidance from senior staff.
  • Effective time management and caseload management.
  • Good problem-solving skills.
  • Working knowledge of clinical outcomes measures and clinical audits.
  • Experience with EMIS, undertaking assessments, and providing holistic care to elderly patients on DN caseload.
  • Management of long-term conditions and palliative care.
  • Aware of safeguarding adults.
  • Search for health needs and work with the core and wider MDT to meet patient needs.

  • Valid Registration with the NMC.
  • Recent PMVA Training.
  • A valid DBS subscribed to the update service.
  • Recent & relevant 6 months of experience within the NHS or Private Healthcare setting.
  • Evidence of immunisations to MMR, Varicella, Hepatitis B, Tuberculosis.
