Trust Nurse Services is looking for enthusiastic and motivated Community Psychiatric Nurses (CPN) to support our NHS clients in Essex. We are offering long-term contracts as well as Ad-Hoc shift work, competitive rates of pay and mileage.
Responsibilities
- Support the CMHT Team Leader in establishing and maintaining effective inter-team and inter-agency communication.
- Provide effective liaison/communication relating to referrals to and discharge from the CMHT across in-patient, primary care and community services.
- Maintain effective joint working with Sector EPMHS, CATTs, EIPs, AOTs and other community-based teams according to local circumstances.
- Ensure that all relevant information regarding client care is communicated to the appropriate persons within and outside the team, bringing to the CMHT Team Leaderβs attention issues of concern regarding practice or workload.
- Ensure effective communications with relatives, visitors, voluntary organisations and the general public, thus encouraging participation in care delivery.
- Respond sensitively to the needs of relatives and carers, often in a highly emotive setting.
- Liaise with other professionals in relation to client care, actively coordinating care planning and review meetings, and case conferences as appropriate.
- Advise others as to the role and practices of the CMHT.
- Maintain positive working relationships and clear communications with team members and other professionals.
Experience
- 6 months of experience working at a relevant level within the community in the UK.
- Has a full UK driving license and access to an insured car daily.
- Able to work within a diverse community of patients and healthcare professionals.
- Effective communication skills, written, verbal and non-verbal, with the ability to adapt communication for individuals with communication difficulties.
- Able to make clinically reasoned judgements when working with a variety of conditions.
- Able to work autonomously, and have the ability to know when to seek appropriate advice/guidance from senior staff.
- Effective time management and caseload management.
- Good problem-solving skills.
- Working knowledge of clinical outcomes measures and clinical audits.
- Experience with EMIS, undertaking assessments, and providing holistic care to elderly patients on DN caseload.
- Management of long-term conditions and palliative care.
- Aware of safeguarding adults.
- Search for health needs and work with the core and wider MDT to meet patient needs.
