  • 5 Salaried GP's
  • 7 Nurses/HCA's
  • 1 Pharmacist Prescriber
  • 1 Practice Manager
  • 20 Receptionists
  • 13 Other administrative staff including leadership team

We also work collaboratively with a team of pharmacists, FCP's and paramedics to provide a multi-disciplinary service to our 16,000 patients.As a training practice we are passionate about supporting our team to learn new skills and develop in their role to better meet the needs of our patients.

Job Summary

We are recruiting an established practice nurse to join our friendly team at Oakwood and Bull Farm Surgery.The successful candidate will work with the Nursing team and GP Partners in delivering preventative nursing interventions and management of long-term conditions to all patients.

Main duties of the job

The post holder is responsible for the delivery of practice nursing services and care for the practice population. They will deliver care within the boundaries of their role, focusing on supporting patients to be healthy, managing of long-term conditions, health prevention and screening activities. They will work collaboratively with the general practice team to meet the needs of patients and supporting the delivery of policies and procedures.Working pattern to be agreed.

The successful candidate will be required to work across both our main site: Church Street, Mansfield Woodhouse, NG19 8BL and our branch site: Bull Farm, Concorde Way, Mansfield, NG19 7JZ.Will require additional enhanced access working hours on a Friday evening until 8pm and Saturday morning on a rota basis.

Job Description/Responsibilities

Clinical practice

  • Assess, plan, develop, implement, and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being.
  • Implement and evaluate individual treatment plans for patients with known long-term conditions.
  • Identify, and manage as appropriate, treatment plans for patients at risk of developing a long-term condition.
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
  • Support patients to adopt health promotion strategies that encourage patients to live healthily and apply principles of self-care.
  • Deliver opportunistic health promotion using opportunities such as new-patient medicals.
  • Provide information and advice on prescribed or over-the-counter medication on medication regimens, side effects, and interactions.
  • Support patients to adopt health promotion strategies that promote patients to live healthily, and encourage principles of self-care
  • Assess and care for patients presenting with wounds and ulcers.
  • Support and advise women requesting information relating to family planning needs
  • Support and manage health needs of women presenting for cervical cytology consultations
  • Implement and participate in vaccination and immunisation programmes for both adults and children.
  • Advise, support and, where appropriate, administer vaccinations for patients travelling abroad
  • Promote and deliver evidence-based care for patients presenting with aural conditions
  • Assist senior practitioners in providing minor-surgery sessions.
  • Be proactive to complete items for Quality and Outcomes Framework (QOF) or any replacement programme, and Enhanced services opportunistically.

Person Specification

Essential

  • Flexibility - the ability to respond positively to occasional changes in shift requirements to meet our contractual needs with regards to extended hours and monthly staff training.
  • Adaptability and time management - the ability to organise and prioritise workload effectively, adapting throughout the working day to changes in appointments, patient needs and clinical priorities while maintaining a high standard of patient care.
  • Proactive and self-motivated - the ability to anticipate patient and clinical needs, take initiative and manage responsibilities effectively, and seeking advice or escalating concerns when appropriate.
  • Collaboration - confident working independently while contributing positively to the wider team, supporting colleagues and communicating effectively to ensure continuity and quality of patient care.

Qualifications

Essential

  • Registered first level nurse
  • Community nursing specialist qualification or equivalent
  • Relevant nursing/health degree

Desirable

  • Independent/ supplementary nurse prescribing qualification

Experience

Essential

  • Minimum two years recent primary care nursing experience
  • Cervical Cytology
  • Immunisation and Vaccination
  • Long term condition management
  • Contraception management
  • Wound care
  • Good communication skills, both written and verbal

Desirable

  • SystmOne trained
  • Experience of completing audits
