The Role You’ll Play - Youth Case Manager
As a Youth Case Manager at Pawnee, you’ll play a pivotal role in empowering children with severe emotional disturbances to thrive. You’ll be the connector—linking families with vital community resources, collaborating with schools, and working closely with treatment teams to develop personalized care plans. Your ability to build strong, supportive relationships with clients and their families will make a lasting impact, helping them navigate challenges and achieve their goals. The Day-to-Day
- Collaborate with the treatment team to identify treatment goals and strategies.
- Coordinate treatment services by building partnerships with schools and community organizations.
- Build trusting relationships with clients and families, guiding them in making informed choices to meet basic needs.
- Accurately document plans and updates in each child’s health record.
- Assist families in maintaining essential benefits to support ongoing rehabilitation.

Requirements
- Bachelor’s degree in a human services field or equivalent work experience.
- Reliable transportation, a clean driving record, and automobile liability insurance.
- Excellent communication skills, both written and verbal.
- Basic computer skills.
Preferred
- Knowledge of the needs and challenges of youth who experience severe emotional disturbances.
- Experience with Microsoft Office.
- Results of criminal and all other applicable background checks must meet the approval of Pawnee Mental Health Services.
