Case Managers assist consumers and families in accessing community resources. Additionally, the case manager will provide symptom management of mental health diagnosis and adjust the patient‐centered plan according to client input and need.
Essential Responsibilities
- Provide services in the community where the consumer may need it, including but not limited to school, work, home, shelter, store, etc.
- Provide outreach, support, referral to services, and connection of resources to the consumer and their family.
- Provide ongoing strengths assessment, eliciting the consumer and their family as active participants in determining strengths, wants, needs, and preferences.
- With input from the client, their family, and other members of the treatment team, develop and document treatment goals and plan‐related services and activities. Monitor and document progress and concerns, adjusting the person‐centered plan as needed.
- Advocate for client rights and community integration. Preserve dignity and confidentiality of each person on the caseload.
- Provide individual therapeutic interventions to assist in assessment of symptom management and skill development.
- Within scope of knowledge, provide resources and education to the client and their family about mental illness or serious emotional disturbances upon request.
- Meet administrative and clinical documentation requirements both in terms of content and timeliness.
- Meet direct service expectations and comply with other productivity standards as established and outlined in policy.
- Attend and actively participate in clinical supervision and program staffing. Provide input to supervisors and other treatment team members regarding clinical and program issues.
- Provide crisis management and/or coordination of emergency services during periods of crisis.
- Drive to community settings for the delivery of services.
- Enter and maintain clinical schedule in the agency electronic health record.
- Maintain and advance knowledge and skills related specifically to the position or to the field of work through regular review of pertinent materials or participation in training events as approved or directed by supervisor.
- Adhere to the standards, policies, and instructions established by PMHS Policy and Procedure and any applicable ethical codes within the field of employment.
- Demonstrate sensitivity to all cultural and ethnic differences.
- Review and comply with the Pawnee Code of Conduct outlined in the Employee Handbook.
- Complete state‐required training specific to the focus of the position. This is required at the first available opportunity.
- Work cooperatively with the Management Team and other members of the staff in sharing resources and expertise in pursuit of the agency's mission.
- Perform all other reasonably related duties as assigned by immediate supervisor or other management personnel.
Qualifications
- Required: Bachelor's degree in a human service field or equivalent work experience, or a combination of work experience in the human services field and education with one year of experience substituting for one year of education.
- Strong oral and written communication skills using the English language.
- A valid driver's license and automobile liability insurance maintained in accordance with Pawnee policy and procedure.
- Basic personal computer skills which enable the entry, retrieval, and use of electronic data.
- Results of criminal and all other applicable background checks and drug screening must meet the approval of Pawnee Mental Health Services.
- Preferred: Knowledge of the needs and challenges of consumers who experience serious symptoms or mental illness and their families.
- Experience with Microsoft Office software.
Salary Description: $20.19 an hour
