PHL have an exciting opportunity for a Triage Clinician to join the team working in our new Neurodevelopmental Single Point of Access service on a full time permanent basis.
The Triage Clinician is responsible for the clinical review and triage of administratively validated neurodevelopmental referrals. Using a structured triage framework, the postholder will assess referral information and supporting documentation to determine the most appropriate pathway, provider, or onward service.
The role is pivotal in ensuring referrals are managed safely, consistently, and efficiently, with appropriate escalation of risk, safeguarding concerns, and complex cases. The Triage Clinician will support effective patient flow by authorising pathway allocation, provider routing, or clinically justified redirection where referral criteria are not met.
Your key responsibilities will be:
- Review administratively validated referrals received into the clinical triage queue.
- Evaluate referral information alongside any available screening tool outputs and supporting documentation.
- Apply the agreed clinical triage framework to determine the appropriate triage outcome.
- Categorise referrals as Urgent, Priority, Routine, or Redirect in accordance with service protocols.
- Ensure triage decisions are clinically appropriate, evidence-based, and clearly documented.
- Identify referrals that require escalation due to safeguarding concerns, significant risk, or acute functional deterioration.
- Escalate urgent referrals immediately to the Senior Triage Clinician and Clinical Manager in line with service procedures.
- Identify referrals involving transition-age young people (17.5+), looked-after children, care leavers, or cases requiring multi-agency coordination.
- Refer complex cases to the Transition and Complex Case Coordinator where additional coordination is required prior to routing decisions.
- Determine whether referrals meet the criteria for acceptance onto a neurodevelopmental pathway.
- Confirm the most appropriate receiving provider for accepted referrals, including: Adult ADHD, Adult Autism, Children’s ADHD and Children’s Autism.
- Authorise referral routing to the appropriate provider.
- Ensure referral decisions support timely access to the most appropriate service.
Requirements:
- Prove experience in a role involving triage
- Minimum of 3 years post-registration experience
- Experience of triage involving mental health
- Good knowledge of neurodevelopmental and its impact on individuals across the lifespan
- Good knowledge of a variety of mental health conditions
- Good general physical health knowledge
