We are currently recruiting for Care Co-ordinators on behalf of a well-established specialist healthcare organisation providing 24-hour specialist care to patients and families across North West London. This is an exciting opportunity for an organised, compassionate and proactive healthcare professional to join a caring and diverse multidisciplinary team and play a vital role in coordinating high-quality, person-centred care. You will work alongside senior nurses and a wide range of health and social care professionals to ensure patients receive timely, coordinated support and, wherever possible, remain in their preferred place of care.
About the Role
- Coordinating referrals and supporting patient care pathways
- Working closely with the nursing team to triage and prioritise referrals
- Acting as a key point of contact for patients, families and healthcare professionals
- Supporting a 24-hour advice and support service
- Facilitating effective communication between healthcare providers
- Monitoring referrals and maintaining accurate records
- Providing information, guidance and signposting to appropriate services
- Supporting efforts to prevent avoidable hospital admissions
- Working collaboratively with NHS 111, ambulance clinicians, GPs, community services and hospice teams
- Contributing to a compassionate, responsive and high-quality palliative care service
