Clinical Support
- Assist in medication reviews and reconcile medicines after hospital discharge and outpatient clinic appointments.
- Provide advice to clinicians on suitable formulary alternatives for out of stock medication, the Pharmacy First Consultation Service, and guidance on medications that may be purchased over the counter.
- Handle patient's daily prescription queries (face to face, online or by telephone).
- Offer guidance on medication use and support long‑term condition management.
- Promote patient self‑care.
- Work with prescribers to ensure adherence to formulary guidelines and manage high‑risk medication monitoring.
- Carry out audits to improve prescribing safety, quality and cost effectiveness.
- Contribute to the effectiveness of our medicines management team by providing support and training as appropriate.
- Maintain accurate records, update patient medication lists and support compliance with CQC and NHS regulations.
- Review ways of working to maximise income from PPA & QOF, and liaise with HCAs to manage appropriate stock levels for emergency use and practice procedures.
Medicines Optimisation
- Support safe, effective and cost‑efficient prescribing by reviewing and managing repeat prescriptions and reducing waste.
- Ensure compliance with practice protocols and best practices.
- Carry out medication‑related audits in agreement with the Clinical Pharmacist and Senior Pharmacy Technician.
Continuous Improvement
- Keep up to date with professional responsibilities and CPD requirements.
- Contribute to ongoing continuous improvement of our operations and patient safety.
- Respect and protect patient confidentiality and information.
