Are you a qualified Advanced Clinical Practitioner (ACP/ANP) from a nursing background seeking a fresh challenge in a forward-thinking, high-performing general practice? At Park Lane Surgery, we are proud to offer an exciting opportunity for an ACP to join our small and friendly team. Main duties of the job:
- Assessing, diagnosing, planning, treating and reviewing patients with both acute and chronic conditions
- Carrying out home visits when requested
- Carrying out part of the physical assessments of Learning disability reviews
- Managing clinical episodes independently including prescribing and referrals
- Taking a lead in unscheduled and emergency care
- Performing clinical procedures and planning care pathways
- Communicating effectively with patients, carers and clinical teams
Shift Schedule is currently: (subject to change)
- Wednesday - 8 hours
- Thursday - 8 hours
- Friday - 9 hours
Park Lane Surgery is a small, but busy GP practice; based in the rural area of Allestree, Derby, with over 8400 patients and 4 GP partners. We are forward thinking and pro-active, always aiming to maintain the highest standards of practice, to deliver excellent care to our patients as well as protecting the safety and well-being of staff. The way that we work is continually evolving, especially so in the last couple of years. We adapt quickly to new ways of working and embrace improvements to practice processes and technology as they become available. We are also part of a forward thinking and dynamic Primary Care Network.
