We are seeking an enthusiastic individual to take on the role of Practice Nurse. The role is to ensure the delivery of an effective Practice Nurse service to the practice population, delivering care appropriately according to practice policy, protocols, guidelines and procedures.
Main duties of the job:
- To carry out a wide range of treatment room duties e.g: venepuncture, new registration checks, blood pressure checks, ECGs, vaccinations and immunisations.
- To run Chronic Disease Clinics such as CHD, Diabetes and Respiratory.
- Offer appropriate lifestyle advice to patients.
- To provide support to the nursing team to ensure smooth running of the nursing service.
Job responsibilities:
- Clinical Practice: Assess, plan, provide and evaluate treatment programmes including, but not limited to:
- Manage and treat patients with long term conditions in line with national/local policies and practice needs with the inclusion of Home Visits.
- Review medication for therapeutic effectiveness in accordance with evidence based practice and national/practice protocols, and work with patients to support adherence to prescribed treatments.
- Support and manage health needs of women presenting for Family Planning and Cervical Cytology Consultations.
- Implement and participate in Vaccination and Immunisation Programmes for both Adults and Children.

- Health & Safety: The post-holder will implement and lead on a full range of promotion and management of their own and others health and safety and infection control as defined in the practice Health & Safety policy, the practice Health & Safety manual, and the practice Infection Control policy and published procedures.

- Training and development: Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development.

- Communication: Communicate effectively with other team members and with patients and carers. Recognise people's needs for alternative methods of communication and respond accordingly.
- Contribution to the Implementation of Services: Apply practice policies, standards and guidance. Discuss with other members of the team how the policies, standards and guidelines will affect own work. Participate in audit where appropriate.
Qualifications:
- Qualified Nurse.
- Experience of GP Practice Nursing.
- Full UK Driving Licence.
- Prescriber Diploma in respiratory.
