In this community nursing role, as a Pediatric Community Nurse RN/RPN you will have scheduled shifts and visits to care for patients in schools and in the comfort of their homes. You will work with patients within pediatric ages with differing complexity of care needs and play a key role in helping patients remain comfortable in their homes for as long as possible. There will also be adult nursing.
Must be willing to travel through the Scarborough geographical area. On call is required and working every other weekend.

What you’ll do:
- Using appropriate assessment tools/techniques to assess patients’ individualized needs
- Executing and continuously monitor care plans
- Engaging patients using a collaborative approach by building rapport and trust
- Maintaining and update professional patient records related to patient encounters and plans

- Registration with the CNO as a Registered Nurse in good standing
- Valid driver’s license and access to a vehicle is required
Compensation Details: $25.76 - $28.54
