Paragon Home Healthcare is seeking Female Complex Homecare Nurse to support a female client who requires ongoing nursing support at home. This role involves caring for a client with a combination of complex long-term conditions, including chronic pancreatitis, hypermobility/Ehlers-Danlos features, chronic pain and neurodivergent needs. She has been in hospital for an extended period, and nurses will play a key part in supporting a safe and well-planned transition home.
Main duties of the job
- You’ll be delivering person-centred, holistic care such as complex medication management (including subcutaneous and jejunostomy-administered medications), bowel care, respiratory support and safe mobility.
- The client benefits from calm, structured communication and consistent routines, and you’ll be supported by clear protocols to guide practice.
- You’ll play a vital role in promoting health, safety and quality of life within the home environment.
- Working closely with our Nurse Practitioner team, you’ll help ensure her care plan is followed smoothly and contribute to a stable, supportive model of care that encourages confidence, independence and overall well-being.

Job responsibilities
- Deliver high-quality, individualised nursing care in the home environment.
- Manage and administer complex medication regimens safely and accurately.
- Provide skilled support for an individual with autism, using a calm, consistent, and person-centred approach.
- Monitor and respond to changes in the clients condition, maintaining accurate care records.
- Work collaboratively with the clinical team, family, and other professionals to achieve the best outcomes for the client.
- Liaise with the clinical team and family to ensure comprehensive care.

- Female carer required due to the need to provide personal care (Equality Act 2010, Schedule 9, Part 1).
