About the Role

As a Care Coordinator, you will:

  • Plan and manage staff rotas using advanced scheduling systems to ensure seamless care delivery.
  • Build strong, trusted relationships with clients, carers, and colleagues.
  • Support and mentor carers, helping them deliver high‑quality, person‑centred care.
  • Work closely with the care manager to ensure care is aligned with each client’s clinical and personal needs.
  • Collaborate with recruitment and HR to onboard staff and train them in our systems.
  • Join our shared out‑of‑hours rota, supporting the team when unexpected issues arise.
  • Be a key part of a passionate and responsive team, solving challenges together.
