The Single Point of Access (SPA) team is seeking a Band 6 Nurse Coordinator to join our dedicated service. This is an exciting opportunity to be at the forefront of patient care coordination across multiple specialities and services.
This role is Par time 28 hours, 8 months Fixed Term (maternity cover).
Main duties of the job
- Significant clinical experience across a range of specialities

Cheshire and Wirral Partnership (CWP) provides health and care services for a population of over one million people, including mental health, learning disability, community physical health and all-age disability care, as well as the provision of three GP surgeries in Cheshire.

Person Specification Qualifications Essential Criteria
- Registered General Nurse (RGN)
Desirable criteria
- Post registration qualification in relevant subject
- Previous experience of facilitating clinical supervision.
Knowledge and Expertise Essential criteria
- Extensive current clinical knowledge
- Evidence of ongoing professional development
- Excellent communication skills, verbal and written
Experience Essential criteria
- A Registered Nurse Adult, currently practising with post-registration experience within a community/primary care setting
- Leadership / management skills
- IT literate with good keyboard skills
Desirable criteria
- Experience of triage / multidisciplinary / interagency working
