Band 6 Registered Nurse

We are looking to recruit a Band 6 Registered Nurse for the Hospital at Home service, providing urgent community response and virtual ward care to West Cheshire residents in their own home. You will be joining a multidisciplinary team that includes GP’s, Advanced Clinical Practitioner, nurses, therapists, pharmacists and clerical officers. This is a permanent post working 30 hours per week across a 7‑day service, between 8 am and 8 pm.

Main duties of the job
- Provide a highly visible clinical profile with leadership and clinical input for a designated clinical caseload.
- Work collaboratively with the wider health, social care and third‑sector partner agencies to meet the needs of patients.
- Ensure patients receive the right care, by the right person at the right time and the right place.
- Place the patient at the centre of all decisions, reinforcing a more streamlined patient journey with minimal avoidable delays and an emphasis on people achieving independence while improving quality of care.
- Provide direction within the caseload in consideration of the impact of current health and social care agendas and local initiatives.
- Aware of local public health community profiles and how this influences the required care for the population.
- Focus on the delivery of person‑centred care.
- Provide evidence‑based care to a defined population through case‑load management and monitoring of care standards.
- Provide clinical leadership, coaching and development of the clinical team where appropriate.

Qualifications and Experience
Essential criteria
- First level registered general adult nurse.
- Evidence of teaching and assessing.
- SPC qualification in District Nursing or equivalent; demonstrating robust evidence of caseload management, community profiling and application of health needs assessment.
Knowledge and expertise
- Proficiency in delivering high‑quality evidence‑based care.
- Demonstrate continuing professional development.
- Effective communication and teamwork within a multidisciplinary team setting.
Experience
- Significant experience working within a community healthcare setting.
- Experience making autonomous decisions.
