Our aim is to deliver an integrated community-based model of mental health care supporting both primary and secondary care services.

Healthier South Wirral Care Network is a collaboration of 6 GP Practices (Sunlight Group Practice, Spital Surgery, The Orchard Surgery, The Allport Surgery, Eastham Group Practice, and Civic Medical Centre), in and around South Wirral who collectively care for over 49,000 patients.

We are looking for an experienced Mental Health Practitioner who is interested in working as part of the GP Multidisciplinary Team in Healthier South Wirral Primary Care Network on a part time basis, 22.5 hours. The role will include:

  • Signposting to other provision in school or the community; supporting access to the graduated approach for potential neurodevelopmental needs.
  • Supporting signposting following assessment by secondary CYP Mental Health Services (CYPMHS).
  • Facilitating transitions out of CYPMHS and building community networks; offering training and upskilling where appropriate.
  • Providing expert advice and support to enable the GP MDT to deliver an effective service for children and young people with mental health needs.
  • Working as part of the GP Multidisciplinary Team within Healthier South Wirral PCN on a part-time basis (22.5 hours).
  • Line management and professional supervision by the CWP Band 7 Access Team Manager; access to training, education and support from CWP.
  • Autonomous and collaborative working style with a focus on improving outcomes for people with mental health issues.

Qualifications and Ideal Candidate:

  • Experienced Mental Health Practitioner.
  • Willingness to work autonomously and collaboratively to improve outcomes for children and young people with mental health needs.

  • The post is hosted via CWP; shortlisting dates and interview dates are provided in the overview.
  • We are a Disability Confident Employer, Rainbow Badge Scheme member, Veteran Aware organisation and Armed Forces Covenant Gold Award holder, committed to inclusive recruitment.
  • All new starters commence in post on one fixed start date each month in line with Prepare to Care (P2C) induction, which offers up to 3 weeks of training.
  • Applicants should review Trust values and reflect them in their application; examples from work experience or personal life may be requested in support of values-based interviewing.
  • The DBS check cost is paid by the applicant; costs can be deducted from salary after appointment. Enrolment in the DBS Update Service is encouraged.
