What You’ll Do
- Communicate regularly with residents and families regarding plans of care, therapy progress, treatment protocols, and resident status updates
- Participate in daily stand-up and interdisciplinary meetings to support seamless communication across departments
- Answer resident and family questions regarding care plans, illness progression, treatments, and services to promote comfort and confidence
- Monitor and assess resident conditions, adjusting care needs as appropriate
- Prioritise and organise daily clinical responsibilities in a high-volume environment
- Complete accurate and timely documentation and charting
- Perform utilisation review activities to ensure appropriate, timely, and cost-effective resident care
- Coordinate care with physicians, ancillary providers, financial services, and third-party payers
- Oversee admissions, transfers, and discharge processes
- Coordinate referrals to and from the facility
- Promote teamwork, collaboration, and professional communication among staff and healthcare partners
- Exercise independent judgement while maintaining professionalism and compassion in all interactions
What We’re Looking For
- Active RN license in good standing
- Minimum of 2 years of clinical nursing experience preferred
- Knowledge of Medicare, Medicaid, and managed care processes preferred
- PCC experience a plus
- Excellent communication, organisation, and time management skills
- Ability to work independently and adapt to changing priorities
- Strong documentation and computer skills
- Commitment to respectful, resident-centred care
