P4 Recruitment are recruiting on behalf of a local charity for a compassionate and committed Care & Support Practitioner to work within a specialist residential service. The home is specifically aimed at supporting people who have experienced, or are currently experiencing, homelessness and who have additional health and social care needs.
You will provide care and support to residents with complex needs including mental health needs and DoLS, acquired brain injury (ABI), dementia, current and past drug and alcohol misuse, advancing ill health and palliative care, alongside many other challenges.
Residents receive person-centred care plans and structured support including scheduled personal care (washing, dressing, laundry), medication administration, daily meals and access to meaningful activities.
This role would suit someone who is naturally empathetic and motivated to help.
