In this role, you will support older adults living independently in their own homes, helping them to remain safe, connected, and confident in their daily lives through regular community-based home visits. You will provide tailored, person-centred support that combines practical assistance with emotional reassurance, building positive relationships and responding to individual needs. The role requires someone who can work independently, communicate effectively, and deliver support with empathy, professionalism, and consistency.
