The Role

  • Hours: 37.5 hours per week
  • Shift Pattern: Monday to Friday, 9:00am to 5:00pm

We are looking for a Senior Support Worker to join our Rotherham Hospital Discharge service. The service offers comprehensive housing-related support to adults on inpatient wards at Swallownest Court. This initiative is driven by our mission to facilitate successful reintegration into the community. We are in search of an energetic, compassionate, and exceptional Senior Support Worker, who is driven by the desire to make significant and lasting differences in people's lives. In this pivotal role, you will be at the heart of the action, coordinating the daily operation of the service. You will also serve as a Keyworker for a dedicated caseload, providing much-needed support in both hospital and community settings, including within people's homes. We are looking for someone who has a passion for social justice and is eager to make a positive impact in their community.
