P3 are looking for an amazing person to help us change lives for people living in Lincolnshire. As Link Worker you will be responsible for the coordination of support through yourself and other agencies. This will involve use of strength-based support planning, detailed safety management plans; trauma informed and psychologically informed approaches. There will be a need to work creatively and consistently with people who may not usually engage well with offers of support.
You will act as a keyworker, encouraging positive and healthy lifestyles and assist individuals to access services that support people facing multiple disadvantages such as homelessness, substance misuse, domestic violence, contact with criminal justice system and mental/physical ill health, often at the same time. The role requires someone with empathy and a practical approach in helping people sustain their tenancies and reach their full potential.
The successful candidate must hold a full UK driving licence. Please note, this post is subject to an enhanced check made by the Disclosure & Barring Services, paid for by P3.
