We have a fantastic opportunity to join our Hospital Discharge team.

If you are self‑motivated and enjoy a challenge where no two days are the same, then this role is for you!

As a Senior Support Worker you will focus on managing a brilliant team of Support Workers in delivering the service to achieve positive outcomes.

You will assist the Service Manager in the overall management of the service and your focus will be delivering the service to provide support that is person centred, holistic and easy to access.

We are looking for a highly motivated, confident and proactive person with excellent communication skills and the ability to manage challenging situations.

The successful candidate must hold a full UK driving licence.
