Oxleas NHS Foundation Trust is seeking aPrimary Care Clinical Lead to join the integrated healthcare team at HMP Guys Marsh. This is a senior clinical leadership role with responsibility for the delivery and development of primary care and clinical substance misuse services.
Working closely with the Head of Healthcare, you will provide expert clinical leadership, support the day-to-day management of services and help ensure that patients receive safe, effective and evidence-based care. You will also contribute to the wider senior leadership team and deputise for the Head of Healthcare when required.
Applicants should be a registered healthcare professional with significant primary care experience, a non-medical prescribing qualification or be working towards one, and hold or be completing a master’s-level Advanced Clinical Practice qualification.

Main duties of the job
You will lead and coordinate the primary care service, ensuring that care is responsive to the needs of the prison population and integrated with mental health, medicines management, GP, dental and wider healthcare provision.
The role includes providing expert clinical advice, supervision and leadership to multidisciplinary staff; overseeing clinical pathways, referrals and care planning; and supporting the effective management of acute presentations, long-term conditions, health promotion, screening, immunisation and substance misuse treatment.
You will promote strong clinical governance through audit, risk management, incident review, evidence-based practice and service evaluation. You will also support workforce planning, recruitment, staff development, performance management and the effective use of resources.
Working with prison colleagues, commissioners, community services and other partner organisations, you will help ensure continuity of care throughout custody and on release.

Detailed job description and main responsibilities
As Primary Care Clinical Lead, you will provide visible, professional and compassionate leadership for the primary care service. You will exercise a high level of professional autonomy, making complex clinical decisions while maintaining accountability for the quality and safety of care.
You will undertake advanced clinical assessment, diagnosis and treatment of patients presenting with undifferentiated, acute or complex needs. Within your professional scope, you will prescribe and review medication, support the management of long-term conditions and ensure patients receive appropriate health promotion, preventative care and self-management advice.
You will lead the development, implementation and evaluation of evidence-based clinical pathways, including chronic disease management, acute care, immunisations, screening, detoxification, rehabilitation and harm-reduction interventions. You will ensure that patients with complex needs receive coordinated, individualised and regularly reviewed care plans.
The post holder will provide clinical and managerial supervision, support appraisal and professional development, and promote a culture of reflective learning and continuous improvement. You will monitor performance, maintain high standards of clinical documentation and ensure staff work in accordance with Trust, prison and national policies.
You will contribute to clinical audit, research, quality improvement and service evaluation, using findings to improve patient outcomes and experience. As a member of the Senior Leadership Team, you will work collaboratively across professional and organisational boundaries and deputise for the Head of Healthcare when required.

Qualifications
- Registered healthcare professional, with current professional registration.

Experience
- Significant post-registration experience within primary care and/or clinical substance misuse services
- At least three years’ management experience within a multidisciplinary healthcare environment
- Experience of service development, change management, clinical audit and quality improvement
- Experience of delivering care within a prison, secure or similarly complex healthcare setting
Skills and knowledge
- Strong clinical leadership and management skills, with the ability to work autonomously and make complex decisions
- Advanced assessment, diagnostic and clinical decision-making skills within professional scope of practice
- Good understanding of clinical governance, risk management, safeguarding, patient safety and evidence-based practice
