Oxleas is seeking a Greenwich Long Term Conditions (LTC) Care Coordinator to join our NHS community teams. This Band 4 role carries a 12-month fixed term contract, full-time hours (37.5 per week) across weekdays, with a focus on proactive, person-centred care for residents with complex needs.
You will manage a defined caseload, coordinate MDT meetings, and implement care plans, supporting patients and families to access services and improve health outcomes.
