We are looking for a community mental health nurse practitioner with exceptional organisational and communication skills. Experience and or interest in facilitating groups/workshops is required. We are seeking a motivated, skilled, and experienced Community Psychiatric Nurse Practitioner to join our Bexley Community Mental Health and Wellbeing Hub.
The Community Mental Health Transformation Programme has led to the development of the Community Mental Health and Wellbeing Hub in Bexley, delivered by a combination of clinical staff from Oxleas, Bexley Mind, Primary Care, Bexley LA, voluntary, third party and local partners. This will enable people to receive support with managing their mental health alongside help with issues including employment, relationships, housing, finances, and engaging in the local community and accessing community assets in Bexley.
The Hub receives referrals from various sources, principally primary care sources including GPs and the London Borough of Bexley. This is an exciting opportunity to be involved with the development of the service and requires a commitment to integrated working.
The post holder will be responsible for undertaking comprehensive telephone and face-to-face assessments of referred patients to identify their needs; they will have to hold a caseload for a short brief-intervention period. The post holders will be working in a supportive and diverse team, where there are opportunities for personal development.
The post holder will be a mental health professional registered with NMC with significant post qualification experience, working either in a single point of access, primary care, or community setting.
