Job Overview
An opportunity to work as a discharge coordinator on Complex Medicine Ward at John Radcliffe Hospital. The post holder aims to facilitate effective, efficient and timely discharge of patients from hospital and hub beds, with emphasis on complex multidisciplinary assessments and case conferences for patients with complex health and social care, palliative care or rehabilitation needs.
Responsibilities
- Facilitate effective, efficient and timely discharge of patients from hospital and nursing home beds.
- Coordinate multidisciplinary assessments and case conferences with nursing, medical, therapy, social work and care home staff, as well as patients, relatives and carers.
- Ensure safe discharge of patients to the most appropriate place and manage patient pathways.
- Work autonomously or as part of a team, demonstrating enthusiasm, organization, excellent communication and IT skills.
- Use own judgement to resolve complex issues, work well under pressure and prioritise workload against constantly changing demands.
- Work alternate shifts, including weekends.
Qualifications

- Experience or understanding of discharge coordination in complex medical settings is desirable.
This role is based at Oxford University Hospitals NHS Foundation Trust, which provides a wide range of general and specialist clinical services in the four hospitals including John Radcliffe Hospital.
