Responsibilities
- Liaison with the directorate pharmacy team lead and the rest of multi-disciplinary team to ensure adherence to guidelines and new product reviews.
- To identify, review and implement processes, policy and procedure that significantly improve the cost-effective use of medication within the clinical area based.
- To provide a comprehensive medication review and clinical pharmacy service to the neurosciences and paediatric clinical areas
- To work closely with the directorate pharmacy team lead for neurosciences and the directorate pharmacy team lead for paediatrics
- To ensure accurate patient medication records are updated and maintained.
- To be responsible for monitoring prescribing and drug expenditure, providing regular evaluated management reports and giving advice on cost containment.
- To work with the neurosciences and paediatrics clinical pharmacy teams to support the pharmacy developments already started and to agree priorities.
- To support the development, implementation and management of givinostat and vamarolone for the treatment of Duchenne muscular dystrophy via homecare and outpatient setting.
