Overdale Medical Practice is seeking an experienced Long Term Conditions Nurse Lead to provide clinical leadership for chronic disease management and improve patient outcomes. You will drive the long-term conditions service across diabetes, respiratory and cardiovascular care, while collaborating with GPs, pharmacists and the MDT to enhance QOF results and service delivery.

The role focuses on leading clinical care, education, audits and improvement initiatives, with a view to high-quality.
