Support Coordinator – Role Overview
We are looking for a proactive and passionate Support Coordinator to join our dedicated team supporting adults with a range of needs, including learning disabilities, autism, mental health challenges and complex support requirements. This role offers a unique opportunity to combine hands‑on support with service coordination and leadership responsibilities, supporting the effective day‑to‑day running of the service. As a Support Coordinator, you will play a key role in empowering the people we support to live independent, meaningful and fulfilling lives. You will provide high‑quality, person‑centred support while coordinating key aspects of service delivery, including support planning, key working, rota oversight and staff guidance. Working closely with the Deputy Manager and Team Manager, you will help ensure services are well‑organised, safe, and outcomes‑focused, and that practice reflects Outward's core values of engage, enable and empower.
Your Responsibilities
- Provide direct, person‑centred support to people we support, promoting independence, dignity, choice and wellbeing in line with individual support plans.
- Coordinate day‑to‑day service delivery, ensuring support is consistent, well‑planned and responsive to people’s needs.
- Carry out key working responsibilities, including completing and reviewing support plans and risk assessments, supporting outcomes and maintaining regular communication with families and professionals.
- Lead on shift coordination and rota oversight, including covering shifts when required to maintain safe staffing levels and continuity of care.
- Support staff practice, providing guidance, mentoring and role‑modelling good practice, including active support and positive behaviour approaches.
- Provide line management and supervision to support staff, as delegated, including inductions, supervisions, performance feedback and development support.
- Carry out routine medication checks, including MAR chart reviews, stock counts and escalation of medication concerns in line with policy.
- Support financial monitoring, including basic checks of service petty cash, receipts and people we support’s finances, escalating discrepancies appropriately.
- Report and monitor maintenance issues, liaising with relevant teams to ensure health, safety and property standards are maintained.
- Ensure accurate, timely and high‑quality documentation, including daily records, incident reports, safeguarding logs and updates on digital care systems.
- Work flexibly across multiple services, including evenings and weekends, to support consistent service delivery.
Requirements
- Experience supporting adults with learning disabilities, autism, mental health needs and/or complex support needs within a supported living or care setting.
- Good understanding of safeguarding and risk management, including recognising concerns, taking immediate action and escalating appropriately.
- Knowledge of health and safety requirements in a 24‑hour supported living environment, including lone working, medication safety and environmental risks.
- Experience of key working, including contributing to support plans, risk assessments and outcome‑focused reviews.
- Ability to support and guide staff in day‑to‑day practice, including role‑modelling good care, supporting shifts and maintaining professional standards.
- Confidence completing and reviewing medication checks, incident reports and basic service monitoring tasks.
- Experience maintaining accurate records and using digital systems for care documentation, rotas and communication.
- Strong communication and organisational skills, with the ability to work effectively with people we support, colleagues, families and professionals.
- Commitment to person‑centred practice, dignity, equality, diversity and inclusion.
Essential Criteria
- Good understanding of safeguarding and risk management within a supported living setting, particularly when supporting adults with learning disabilities, autism, mental health needs and complex health or behavioural presentations.
- Knowledge of health and safety requirements in a 24‑hour supported living environment, including lone working, moving and handling, infection prevention and environmental risks within shared and self‑contained accommodation.
- Clear understanding of confidentiality, GDPR and data protection principles, with the ability to manage sensitive information, records and digital systems appropriately.
- Experience maintaining accurate records and using digital care, medication and rota systems to support safe and consistent service delivery.
- Ability to work confidently within a multi‑disciplinary framework, liaising with families, health professionals, social workers and internal teams as required.
- Strong organisational and communication skills, with the ability to prioritise tasks, support staff practice and contribute to the smooth running of Desmond Court.
- Commitment to person‑centred practice, positive risk‑taking, dignity and promoting independence in line with individual support plans.
- Access to a smartphone capable of receiving SMS messages and installing authentication applications, required for secure access to organisational systems used at Desmond Court.
- Right to work in the UK. Outward will not provide visa sponsorship.
Desirable Criteria
- NVQ Level 2 or 3 in Health and Social Care (or equivalent).
- Additional training in PBS, safeguarding or housing‑related legislation.
